Online Rec Registration - Fall 2010 - Step 1

Thank you for using MEGU online registration. This is a multi-step process, so please do not stop until you see the final "Registration Complete" step.

Online registration MUST be accomplanied by a PayPal payment. If you are not comfortable with making an online payment, you may instead print and mail in the following Fall 2010 Recreational Program Registration Form with check (and copy of birth certificate if we don't already have one).

INFORMED CONSENT/INSURANCE NOTICE

FYSA RECOMMENDS THAT PLAYERS NOT REGISTER TO A TEAM WHOSE AGE GROUP EXCEEDS THE PLAYER'S NORMAL AGE. It is FYSA's policy that all players compete at a level they are capable of both physically and developmentally. For a player to move up more that one normal age grouping will require approval from the affiliate's director of coaching or agent of record, and the FYSA Director of Coaching.

INSURANCE NOTICE:  All injuries must be reported within 90 days of the date of the injury.

INFORMED CONSENT:

  • I, the parent/guardian of the registrant, agree that we will abide by the rules of MEGU Soccer, the state association (FYSA), and all its affiliated organizations.
  • My/our child wishes to participate in soccer during the season of this registration.
  • I/we realize risks are involved in my/our child's participation.
  • I/we understand that the risk to my/our child includes full range of injuries from minor to severe, and the result could be death, paralysis, or other serious, permanent disability.
  • I/we accept this risk as a condition of my/our child's participation.
  • I/we understand that MEGU is a volunteer organization; MEGU requires each player to pay a $15 volunteer fee at the time of registration. Each family must volunteer a MINIMUM of two hours per player registered in order to be eligible for a refund of the $15 volunteer fee. For more details, see the Refunds section below.

REGISTRATION DETAILS

Fall 2010 REC REGISTRATION FEES (includes refundable $15 volunteer fee):
U6-U8 player $90.00
U10-U19 player $115.00
MULTI-CHILD DISCOUNT
First child pays full registration fee.
Second and third child within the same family receives $20 off the registration fee.
Fourth and more receives $40 off the registration fee.
DEADLINE:
Early registration deadline is July 31 2010 for all age groups.
Registration is not complete until full payment and documents are received.
Late registration rates increase $15, and will be considered as space becomes available.

REFUNDS:

  • MEGU will refund your registration fee if unable to provide an age appropriate team for your child. Board must approve all other refund requests. There will be a $15 charge for withdrawal after players have been registered with FYSA.
  • The $15 volunteer fee, included in your registration payment, will be refunded upon completion and verification of volunteer hours. Refunds must be approved by MEGU's president, volunteer/concessions coordinator, and/or recreational registrar.

ELIGIBILITY:

  • Players at least 4 and no older than 18 years old by August 1, 2010 are eligible to register providing they are not currently registered or under suspension/ bad standing with any USYSA affiliated association, club, or team.
  • U6 includes players that are 4 by 8/22/10, and not yet 6 by 8/1/10

TEAM PLACEMENT:

  • In order to provide balanced teams, MEGU registrar will place players randomly within their age group.
  • If there are any special accommodations needed for your child, please note on application.
  • Teams will be formed approximately 2 weeks prior to 1st game, coaches will contact players at that time.
  • You will not be contacted to confirm registration prior to that time.

SEASON DATES:

  • August 22nd through November 7th, 2010.

REGISTRATION:

  • Includes FYSA/BYSL fees, uniform (shirt, short, socks), referee, field use, maintenance
  • Trophies, pictures, other equipment are not included
  • Your check may not be deposited until start of season

ADDITIONAL NOTES:

  • New players (players who have not registered with a Florida Youth Soccer Association (FYSA) club in the last two years) must submit a copy of birth certificate by mail (see address on print/mail registration form). Registration is not complete until certificate is submitted!
